Function:

The candidate will be facilitating the training and program development for providers in primary care settings.

Responsibilities:        

  • To work collaboratively with community partners to operationalize, implement, and evaluate integration activities while establishing linkages between partners;
  • Deliver presentations to management teams outlining the implementation plan, program components, partnership agreements, as well as program results;
  • Provide educational presentations and training workshops to Primary Care Clinicians;
  • Collaborate with other members of the team, other departments within Prevention & Rehabilitation (i.e., in-patient smoking cessation) and within our community (i.e., Public Health) to facilitate program development and integration;
  • Develop and maintain, through regular phone calls and face-to-face meetings, working relationships with key players of partner organizations;
  • Coordination and evaluation activities related to our program;
  • Provide feedback and reports on progress of assigned partner sites;
  • Travel as necessary within Ontario to provide on-site training and facilitation activities;
  • Participate in the development of manuscripts and professional presentations to facilitate knowledge transfer;
  • Completion of tasks and duties delegated by the Manager, Associate Director or Chief.
  • Support quality improvement work for Primary Care Project (print materials for clinicians and patients, feedback reports, video, and one on one or group consultations)

Basic Requirements:

  • Bachelor Degree in a health-related discipline;
  • Minimum of 6 years of related experience;
  • Experience in working with community outreach activities;
  • Previous experience in program/service development and implementation;
  • Strong understanding of principles of behaviour change, knowledge transfer and evaluation;
  • Demonstrated ability to work within a multidisciplinary team;
  • Demonstrated presentation skills;
  • Excellent written and oral communication skills;
  • Excellent time-management skills and ability to prioritize workload;
  • Ability to function and participate as a central member of a multi-disciplinary team;
  • Ability to work under pressure on several projects concurrently and continue to meet deadlines as required;

Preferred Qualifications:

  • Demonstrated knowledge of smoking cessation literature;
  • Previous experience in program implementation in health-care/wellness settings
  • Current membership with professional association
  • Proficiency in speaking and comprehending both English and French (A- level)

Intensive training will be provided to the selected candidate in order to prepare for the position.
