Referring a Patient

Physicians can refer patients to the Heart Institute in the following ways. You may also do so by contacting one of our staff physicians.

FrancoForme
A French language cardiovascular prevention and rehabilitation program directed to the Francophone population of the Champlain region. FrancoForme accepts referrals of patients with known cardiovascular disease as well as patients with risk factors for primary prevention.

Pulmonary Hypertension Clinic
The Pulmonary Hypertension Clinic brings together cardiologists, respirologists and nurses at the Heart Institute and from The Ottawa Hospital to provide comprehensive patient care.

Quit Smoking Program
Open to patients and the public. Quitting smoking is the single most important thing a person can do for their health.
