In addition to the rehabilitation programs offered at Heart Institute, there are a number of community program options available to residents living in the Ottawa-Carleton area and surrounding regions.

Community Physical Activity Resources

Heart Wise Exercise helps you find physical activity programs that are safe and appropriate for your particular situation.  These programs meet criteria set by the University of Ottawa Heart Institute and community partners. Find hundreds of locations across Eastern and Southern Ontario.

Regional Cardiac Rehabilition Resources

Pembroke Regional Hospital Cardiac Rehabilitation Program

  • Duration: Three to six months
  • Modeled after Heart Institute on-site program
  • Supervised on-site, twice-weekly exercise sessions
  • Education sessions
  • Medical assessment
  • Referral to a dietitian or social worker as needed
  • Case-managed home program also available

Hawkesbury and District General Hospital Supervised Program

  • Brief exercise program also available
  • Education sessions - bilingual
  • Referral to such services such as diabetes clinic, smoking cessation and telemedicine clinic in collaboration with UOHI for Heart Healthy Workshops and Telehome monitoring for heart failure patients
