The Heart Function Clinic is a specialized outpatient cardiac clinic focused on the care of high-risk heart failure patients and patients who may be candidates for advanced heart failure therapies like the durable Ventricular Assist Device or VAD, and Heart Transplant.
Referrals
Your physician may send your referral to the Heart Function Clinic for general heart failure assessment and optimization (ON patients only) or for evaluation of advanced heart failure therapies – VAD/ Transplant (ON/ QC/NL/NU patients).
They should include your complete medical history, an updated medication list, and any relevant testing.
Your physician’s office may fax the referral to Heart Function Clinic at 613-696-7159.

What to expect
Medical and Nursing staff provide:
- Medical assessment and follow-up
- Personalized testing and treatment to best meet patient needs
- Assistance with managing medication and therapies for heart failure treatment
- Ongoing patient and family education about the disease process, diet, lifestyle, self-monitoring, and self-assessment
- Optimization of quality of life, symptom management and prevention of hospital admissions
- Ongoing support through follow-up visits and telephone monitoring
