Function

The candidate facilitates the delivery of the Ottawa Model for Smoking Cessation (OMSC) program to support healthcare organizations with assisting their patients with tobacco and nicotine addiction. They do so by working with hospital, primary, and specialty care settings to implement and deliver evidence-based smoking cessation best practices as part of routine patient care. They act as facilitators to ensure successful implementation of the program and provide ongoing support for quality improvement activities and sustainability.

Responsibilities

  • Assist in the implementation, delivery and maintenance of activities outlined in program work plans
  • Apply evidence-based best practices through the use of facilitation, problem solving, and system change approaches
  • Collaborate with community agencies and other team members
  • Develop and maintain working relationships with key players in affiliated institutions and in other clinical environments
  • Organize and deliver elements of training to tobacco dependence specialists, health practitioners, and front-line staff
  • Collect data, maintain records, provide reports and support evaluation activities
  • Prepare and deliver presentations and conduct meetings
  • Travel as necessary to institutions and other clinical settings in Ontario and throughout Canada
  • Provide feedback and program progress reports to the Program Manager
  • Collaborate with other members of the team, other departments within Cardiac Prevention & Rehabilitation and within the community to facilitate program development and integration
  • Participate in the development of manuscripts and professional presentations to facilitate knowledge transfer
  • Support quality improvement work for the OMSC program
  • Complete projects, tasks, research and other duties delegated by the Smoking Cessation Director, Chair, and Program Manager

Basic requirements

  • Bachelor degree in health-related discipline
  • 3 to 5 years of related experience
  • Experience in working with community outreach activities
  • Demonstrated ability to work within a multidisciplinary team
  • Strong written and oral communication skills
  • Demonstrated presentation skills
  • Excellent time management skills and ability to prioritize workload
  • Previous experience in program implementation in health-care/wellness settings
  • Superior problem solving and analytic skills
  • Excellent interpersonal skills
  • Works effectively and efficiently in a fast-paced, deadline-driven environment
  • Ability to work under pressure on several projects concurrently and continue to meet deadlines as required

Preferred qualifications

  • Registered health professional
  • Demonstrated clinical research experience
  • Demonstrated knowledge of smoking cessation literature
  • Previous experience working in tobacco cessation and/or health promotion
  • Proficiency in speaking and comprehending both English and French
